Due to the size of this form, we recommend completing it on a desktop computer.
This is our standard Enduring Power of Attorney form which will ask a range of questions about you, your attorney and your wishes.
The important things here are knowing your own wishes and what you are comfortable with your attorney doing on your behalf if you were to lose capacity.
If you are not sure about something, you can leave us a note in the box and one of our team will ring to discuss it with you.
Please note that with Enduring Power of Attorney, you do require independent legal advice to sign them. This means once the forms are ready, we will need to organise for you to see either one of our team or a local lawyer.
Please make sure you complete a Customer Due Diligence form before starting your EPA. We cannot continue any work if this has not been completed and accepted.
You can complete it here.
You can appoint up to two people.
Maximum number of attorneys reached.
Maximum number of successors reached.
After you submit this questionnaire you will need to meet with a lawyer to finalise your EPA. You can make an appointment here, or phone our office on 06 368 9239.
